Degree and area of specialization:

Masters in Social Work required.

License/certification:

WI License or eligibility for licensure in the state of Wisconsin.

Minimum number of years and type of relevant work experience:

- A minimum of 2 years of family therapy or counseling experience with a diverse clientele covering the age spectrum. 5 years of experience for appointment at the senior title.
- Previous experience with educational responsibilities for health profession learners preferred.
- Previous clinical work collaborating with medical professionals preferred.

Principal duties:

The Clinical Social Worker's duties consist of:

A. Clinical Responsibilities (80%):

1. Consult with providers to identify and manage patients seen at the Eau Claire Family Medicine Clinic.
2. Provide co-therapy to patients with the residents and faculty for problems commonly encountered in Family Medicine
such as: depression, anxiety, and adjustment disorders.
3. Jointly provide education to patients and families on topics such as: substance abuse, parenting, developmental stages,
and life transitions.
4. Participate in the Geriatric Assessment Clinic as assigned.
5. Provide referral information and assist with facilitation of the referral for the patient and families to maximize the community
resources in areas such as substance abuse programs, children and family and aging.

B. Educational responsibilities (20%).

1. Co-coordinate and teach components of the Behavioral Health curricula.
2. Participate in the Geriatric Assessment sessions as assigned.
3. Serve on clinic and residency committees as available.
4. Provide case consultation to develop the residents' knowledge and skills in the areas of human behavior and mental health.
5. Assist with the professional development of the residents.
6. Facilitate communication between the residents, staff, faculty, and between clinical sites.

Additional Information:

- Employment contingent upon results of criminal and caregiver background checks.
- Prefix assignment determined by years of relevant work experience.
- Work schedule 8 AM to 5 PM.
- Position is located at the UW Health Eau Claire Family Medicine Clinic, 617 W. Clairemont Avenue, Eau Claire, WI.
